The Santa Ana winds bring in a variety of weirder-than-usual emergencies as the 118 respond to a family dog retrieving a severed hand and a young woman sleep-driving into the station. Bobby worries about his AA sponsor; Denny questions Hen and Karen about his biological parents; and Chimney thinks he’s found the perfect house for him and Maddie.
The 118 investigates a series of calls that have to do with fantasies. A teenager at a Game of Thrones-esque carnival gets several bees stuck in her knight suit. A man fears he may have accidentally killed his bad boss after fantasizing about it. A couple attempts to have sex on top of the fire truck, but one of them slips off. May meets an old friend, Darius, and learns that one of his roommates, Erik, is a misogynist. Erik threatens Darius’ life, but with Athena’s help, May is able to defuse the situation, and Erik is arrested. Eddie wonders if Christopher has a crush following a conversation about a school party. Chimney and Maddie try to find a new house for them and Jee-Yun, but are ultimately unsuccessful.
Felisa, a famous livestreamer, is crushed by a chandelier upon opening and putting on a bracelet from Pompeii. The 118 begin to believe that the bracelet carries a curse as numerous incidents with Felisa occur while she is wearing it. Eventually, Athena catches the suspect, who is a counterfeiter who was trying to steal his bracelet back and continue scamming more people. Buck is able to successfully donate his sperm to Conner and his wife.
On Take Your Child to Work Day, an explosion happens in the lab where Karen is taking Denny, forcing the 118 to go in. Backstory is shown of Hen and Karen’s relationship from start to present, and it’s revealed that Chimney was responsible for setting up their first date. Karen nearly dies from a wound she sustained during the explosion, but Hen and the rest of the team save her. Hen ultimately decides to give up her dream of being a doctor and stays in the 118 to continue supporting Karen and Denny.
Athena investigates a suspicious string of robberies after two home invasion incidents occur at the same house within 24 hours, meanwhile the 118 respond to a D-I-Y project gone wrong. Hen completes her medical practical and passes to now officially become a doctor where she spends one last moment with her fellow colleagues at the 118 and Bobby and Athena must find a new home for their less than well-behaved rescue dog Hoover, who ends up at every members home at the 118 and after dozens of failed placements, Bobby gives the dog to Sharlo, a home invasion victim.
The 118 responds to a man trapped under tree roots while birdwatching with his sister, after which Bobby returns to the station and comforts Hen over her struggles during her time as captain. Buck’s old friend Conner asks him to be his sperm donor. Eddie gets the cold shoulder from Christopher after punishing him for skipping science club to hang out with his friends, and Chimney, Hen, and Ramon all explain that Eddie is simply being too overprotective of his son. Chimney treats a drunk driver at an accident scene who speeds off with him in the car after suddenly regaining consciousness, and Buck is forced to chase them down on a bicycle. After Buck confides in her about being a sperm donor, Hen decides to try med school again, and her professor agrees to give her a second chance. A young girl calls Maddie when her abusive father shows up to take her away from her mother, who is an illegal immigrant. Maddie almost fails when she misinterprets the address, but Noah helps her and together they save the girl. Eddie finally acknowledges that Christopher needs more independence and they make amends. Athena returns home after finding a caregiver for her father. Buck agrees to donate his sperm to Conner.
The 118 race to the rescue when a structure collapse at a “happiness convention” traps people under rubble. While on FaceTime with her mom, Athena’s father suffers a stroke and crashes through the wall of his house. Hen stretches herself too thin with test prep and her new responsibilities at the 118, while Maddie tries to help new recruit get his bearings at the call center.
When a blimp suffers mechanical failure and its engine catches fire, Athena and the 118 must rescue victims both inside and outside a packed sports stadium. Bobby and Athena drop off May on her first day of college and discuss honeymoon plans, while Maddie and Chimney go to couple’s therapy.
Another day in the life of the 118, as its members race to rescue a cliffside wellness retreat guru after his followers turn on him. Then, they respond to an emergency call at an upscale hair salon. Meanwhile, Maddie contemplates returning to work, Eddie and May make decisions on their future, as do Buck and Taylor, and the 118 gathers for a surprise wedding.
After a mysterious death, Hen and Chimney put their lives in danger when they suspect that someone is playing God to make themselves look like a hero. Meanwhile, Eddie visits Texas, where he attempts to reconcile with his father.
